• page 25, Exercise 2.30. The second constraint should be "x+z>=0" instead of "z>=0". (Thanks to Igor Molybog).
• page 49/50, Theorem 3.3, part (2). The inertia should be "(k,2d-2k,k)" instead of "(k,2d-k,k)". (Thanks to Michael Smith and Nathan Krislock).
• page 451, First line of Section A.2.2, "hold" should be "holds"
• page 452, in the last displayed equation of Section A.2.3, it should be "<= u*" instead of "=u*"
